Long Term Care

At Heritage Pointe Rehab we understand that some patients require a higher level of care. We listen to our patient’s wants and needs and figure out solutions that best suit them. Our residents are never alone as we have rounded-the-clock nurses to help assist them. At our facility we never want our residents to lose independence. We encourage independence by hosting events, group activities, and family visits. We want our center to feel like home for our residents, which is why we offer many services in our long-term care program.
